Summer Camp & Back-to-School Forms: Plan Ahead

As summer programs, daycare enrollment, and back-to-school preparations begin, our allergy practice experiences a significant increase in requests for medication and emergency action forms.

If your child carries an inhaler, epinephrine auto-injector (EpiPen), or requires allergy/asthma medication administration at school, camp, or daycare, please submit forms as early as possible.

Important Reminders

Turnaround Time

Please allow approximately 5–7 business days for completion of forms. Increased seasonal demand may occasionally extend processing times.

Annual Follow-Ups Matter

For patient safety and accurate medical documentation, children must be current with recommended follow-up appointments in order for forms to be completed. If your child already has an upcoming appointment scheduled, we may be able to complete forms based on that visit.

Updated Weight Required

An updated weight is required for all epinephrine auto-injector forms to ensure the appropriate dose is prescribed. The transition to the adult-dose auto-injector generally occurs at 55 lbs.

Medication Changes

If your child’s medications, doses, or treatment plans have changed since their last visit, an updated appointment may be needed before forms can be signed.

Self-Carry Authorization

Requests for students to self-carry medications, including inhalers and epinephrine auto-injectors, are generally considered for middle school-aged students and older, based on provider discretion and school requirements.

Planning Ahead for the School Year

If your child has an appointment now but forms are not needed until August or the start of the school year, we can often complete forms with appropriate effective dates for the upcoming school year.

Before You Submit Forms

To help us complete your child's paperwork as quickly as possible, please include:

✓ Your child's current weight

✓ The antihistamine you would like listed on allergy action plans (if applicable)

✓ Whether you are requesting self-carry authorization

✓ Blank school, camp, or daycare forms

✓ The date the forms are needed

How to Submit Forms

Forms can be:

• Uploaded through the patient portal

• Faxed to our office

• Dropped off during business hours
